Project Change Care Package Program

Our care packages are hand-picked and packaged with love and encouragement for children and adults with Fibrous Dysplasia who have recently been diagnosed with the disease or have had a recent surgery for this disease. We know how tough a diagnosis or surgery can be on your family, so we want to send a gift to put a smile on your faces!

If you or your family member/friend has recently been diagnosed or have had surgery and you would like to request a care package, please fill out our request form.

  • Anyone who has recently received their first diagnosis of Fibrous Dysplasia
  • Anyone who has had a Fibrous Dysplasia-related surgery
  • International applicants may apply

  • Care Packages can be applied for a patient within the first year of receiving their initial diagnosis of Fibrous Dysplasia.
  • Care Packages can be applied for in the first month post-surgery. Any requests after one-month post-surgery may not be fulfilled.

The following people (must be 18 or older) can complete the care package application:

  • Patient
  • Family member of patient
  • Friend of patient

  • No, Project Change can only send Care Packages to patients once the surgery is completed.
  • While we would love to be able to send a Care Package to every surgery patient, we ask that requests not be made for surgeries older than 1 month. This helps us to be able to supply more gifts to current surgery patients.
  • Also, Project Change is only able to send one Care Package per patient per year, regardless of amounts of surgeries.

Fibrous Dysplasia is a rare bone disease that affects each patient differently. Just like a zebra, no 2 patients are alike in their diagnosis or treatment plan.

Project Change has selected the zebra to represent our care package program.

  • Send a Zebra to a FD Patient: Donate $25 to provide a FD patient with a post-surgery care package. We will ship the package directly to the patient with a hand written note.
  • Adopt a Zebra: For $35, we will ship you a sweet Project Change zebra in appreciation for your donation and the remaining proceeds will be donated to our care package program.
  • Make a Donation of your choice: Every donation is appreciated!
